PRACTICE POLICIES

Confidentiality

  • We adhere to Ontario’s Personal Health Information Protection Act and have implemented robust measures to safeguard your electronic medical records from data loss and unauthorized access.
  • The privacy of our patients is our top priority. We will not share any personal health information without the explicit consent of the patient or guardian, except as required by law.
  • our administrative staff will not disclose information about medical notes, immunization records or test results over the phone unless instructed to do so by the physician.

Appointments

  • Our consultations and primary care visits are by appointment only.
  • Appointments should be made by phone or e-mail.
  • We require 24 hours’ notice to cancel or re-schedule a doctor’s appointment.
  • Late cancellations will be subject to a fee of $185 CAD for primary care visits and 185 for pediatric consultations.
  • If you are more than 15 min late for your appointment, we may need to reschedule to a later time or date, depending on availability. A no-show fee may apply.
  • Accordingly, a missed appointment fee will apply if a patient fails to show up for a scheduled appointment. The fee is $85 for primary care visits and $185 for pediatric consultations.

Insured services

  • A valid OHIP card is required for all appointments. If you do not have a valid card, you will be billed directly for the visit
  • If your baby is a newborn, please remember to register for a permanent OHIP card as soon as possible. The hospital-issued OHIP number is temporary.
  • For patients without OHIP coverage but with private insurance, we will bill you for services and provide an invoice for insurance reimbursement.

Non-insured services

  • Not all medical services are covered by OHIP.
  • If a service is not covered by OHIP, the patient or guardian is responsible for payment.
    We will bill you for non-insured services and provide an invoice that you can submit to your insurer for reimbursement.
  • Our fees are set in accordance with the Ontario medical association’s physician fee guide for uninsured services.

Privacy policy

We collect personal health information about you and your children. This information may be used by healthcare professionals to provide care, manage administrative tasks, and comply with legal requirements. Your personal health information will only be shared with other healthcare providers who are directly involved in your child’s care as permitted by law.
